Passport Dental℠: Find Dental Care When Abroad

Passport Dental helps Delta Dental of Connecticut members find dental care outside the United States through the AXA Assistance worldwide referral network of curated dentists and dental clinics. Most members can receive both emergency and nonemergency care, subject to their plan benefits.

How to find a dentist when abroad

Contact AXA Assistance for a referral through an English-speaking operator, available 24/7:

Always be sure to identify yourself as a Delta Dental of Connecticut member.

What to know about coverage

Most Delta Dental members can receive both nonemergency and emergency dental care through Passport Dental. However, dentists referred by AXA Assistance are not Delta Dental participating dentists, so non-participating (out-of-network) coverage limits under a member’s Delta Dental plan will still apply.

How to get reimbursed for a foreign claim

You will need to pay the dentist directly for the treatment and then submit a claim to Delta Dental of Connecticut after returning home. Reimbursement is based on the terms and conditions of your existing Delta Dental coverage.

Before you leave the dental office, ask for an itemized bill that includes:

  • The dentist's name and address, including the country
  • The date of service
  • A description or code for each service performed (including what tooth or teeth treated)
  • The charges and whether they are listed in U.S. dollars or local currency

Helpful tips for submitting a foreign claim

  1. Complete the top section of the ADA claim form (numbers 1-22).
  2. Attach the itemized bill and any supporting documentation (it’s okay if it’s not in English).
  3. Mail the claim to:
    Delta Dental of New Jersey
    P.O. Box 16354
    Little Rock, AR 72231
