Requirements of Grantees
 
  • Grantees must include a copy of their current valid Internal Revenue Service tax exemption determination letter (must be a 501(c)3 non-profit or a government entity) or most current 990 form.

  • Grantees must complete the Grant Application provided by our Foundation, and answer all questions.

  • Grantees must sign and return our Foundation's Grant Agreement before funds are disbursed.

  • Grantees are required to complete all answers to an Outcomes Report that will be supplied to them. The Outcomes Reports are due at the six-month mark of the grant period and 45 days after the end of the grant period.

  • Grantees must allow Foundation Board members to make a site visit at their facility at a mutually convenient time.

  • Grantees must file an application annually, even when they are continuing programs from year to year.

2012 Grant Cycle - Underserved

Please note that our funding priorities are different than in prior years. In order to be considered for a grant in 2012, your organization must apply for a grant that focuses on providing dental care to at least one of the following populations (click on each population below for additional information):

The Round One grant application is due on or before Friday, August 19, 2011.

The Delta Dental of New Jersey Foundation Will Not Fund:

  • Individuals

  • Entities that do not have a 501(c) tax exemption

  • Projects, programs, and/or services non-dental related

  • Projects, programs, and/or services located outside of New Jersey or Connecticut

  • Non-oral health related promotional items such as balloons and t-shirts

"The Paterson Public School's Free Dental Clinic was able to hire a full-time dentist due to the generous grants received from the Foundation in 2003, 2004, and 2005. This makes Paterson's school system the first school system in New Jersey to have a full-time clinical dentist employee treating students in a school-based clinic that operates 210 days a year. Without the support of the Foundation, children would be on a long list waiting for appointments while the district hired a dental consultant 12 hours each week to provide dental treatment. Many of the children who attend the dental clinic have not had any dental treatment until the first emergency visit to ease pain. The Foundation grant allows the District's clinic to operate as a regular dental office with regular recalls to follow-up with preventive care once emergency and restorative visits are complete. Not only did the Foundation provide financial support for the dentist, they purchased an x-ray processor and various dental instruments necessary for care.
